Get ready to gear up and get cleared for an incredible season at Colony High School! 

Here’s your playbook for joining our team:

1️⃣ Get Your Physical Form: Download and Print the form below (all 3 pages). Remember, only MD/DO exams are accepted (sorry, no chiropractic exams).

2️⃣ Visit AthleticClearance.com: Head over to www.athleticclearance.com.

3️⃣ Register: Select California, then find Colony High School and choose your activity (that’s football, of course!).

4️⃣ Complete Registration: Fill out all necessary info including emergency contacts, medical history, and district procedures.

5️⃣ Double-Check Everything: Make sure every box is checked on the Athletic Clearance web page.

6️⃣ Print Confirmation: When you’re all set, print out the confirmation email from Athletic Clearance.

7️⃣ Final Steps: Bring both your printed confirmation and completed physical form to Mr. Strout,  Athletic Trainer, for approval and final clearances.
